The Department of Environment, Science and Innovation (DESI) will use the information collected on this form to investigate a complaint against an auditor made under section 574H of the Environmental Protection Act (EP Act). The complaint will be investigated in accordance with Chapter 12, Part 3A, Division 5 (Complaints) of the EP Act. If you consent to the disclosure, the information may be disclosed to the auditor in the course of investigating the complaint. The information provided on this form will not otherwise be used or disclosed unless required or authorised by law.
